Congratulations on booking your first appointment; I am excited to work with you! Please find a list of commonly asked questions and further information required to get us started.
DURATION: The initial appointment will run for approximately 1 hour in length. Please arrive 5 minutes early to ensure we can get started on time.

WHAT YOU NEED TO DO:
In preparation for our meeting, I’d love for you to complete the following:
- 1 x Completed Questionnaires – A link to an ‘MDA Questionnaire’ will be emailed to you shortly. This email will be from Bioconcepts, so please check your junk mail carefully. Please set aside 10 minutes to complete this. The results of these questionnaires will give me an overview of your case before we begin.
- A 4 DAY food and symptom diary using either a copy of the PDF attached (to print and complete by hand) OR an online recollection via the app My Symptoms where it can be emailed directly to me prior to our appointment (see PDF instructions below).
- Please email through a PDF copy of any previous blood or functional testing results you have completed in the last 18 months. If you don’t have a copy, please contact your existing practitioner’s reception. If you’ve not had any blood work done, not to worry we can discuss additional testing at the appointment. PLEASE NOTE: Photographs or images of your existing test results can not be accepted. If you don’t have a scanner, feel free to use an app like CamScanner to send through results.
- In addition, I’d recommend you discuss your family health history i.e. mental, physical, history of autoimmune, reproductive challenges and cancer with your mother, father and grandparents before the appointment. This can help us in more ways than one 🙂
All of the above information will help me piece together an understanding of your clinical picture prior to our first appointment.
