APPOINTMENT DETAILS

Congratulations on booking your first appointment; I am excited to work with you 🙂

Please find a list of frequently asked questions and further information required to get us started.

DURATION: As discussed, the initial appointment will run for 1 hour. You can read more about my appointment structure here.

LOCATION: This appointment will use the software Zoom. The link to our online meeting room is located in your initial appointment confirmation email. If you’ve misplaced it, I’ve also added it to your reminder message, so don’t worry.


WHAT YOU NEED TO DO: 

In preparation for our meeting, I’d love for you to complete the following:

  • 2 x Completed Questionnaires – A link to both a ‘Health Appraisal Questionnaire’ & ‘MDA Questionnaire’ will be emailed to you shortly. Please set aside 30 minutes to complete these. The results of these questionnaires will give me an overview of your case before we begin.
  • A 4 DAY food and symptom diary using either a copy of the PDF attached (to print and complete by hand) OR an online recollection via the app My Symptoms where it can be emailed directly to me prior to our appointment (see PDF instructions below).
  • Please email through a copy of any previous blood results or functional testing (stool tests, salivary hormone profiles etc) you have completed in the last 24 months. If you don’t have a copy on hand, you can simply call your practitioner’s reception and arrange to collect a copy.  If you’ve not had any blood work done, not to worry we can discuss additional testing in the appointment if required.

All of the above information will help me piece together an understanding of your clinical picture prior to our first appointment. As such, the sooner I receive this information the better ?
